If you have recently changed your last name, then you will need to notify your state board and/or NASBA so that any current and future Notices to Schedule (NTS) will accurately reflect your new name. Failure to notify your state board about your name change could mean that you are denied entry to your exam, since the name on your NTS and identification need to match.
